How do I obtain DHS records and/or get a copy of something from my file?

You can request DHS records by filling out our online form below. As these are confidential and non-public records, please do not reach out directly to our DHS employees. If you have an open Dependency and Neglect case, please reach out to your caseworker for guidance.

How can I get a replacement EBT card?

To get a replacement EBT card, call 1-888-328-2656.

Need to pick up an Electronic Benefits Transfer (EBT) card? Once your application is processed, you can pick up your EBT card at any of our three locations. You can also make an appointment for pickup, or we can deliver the card to you via priority mail. Call 720-944-4DHS (4347) for more information, or to make an appointment.


How can I find the balance on my EBT card?

You can check the balance on your EBT card by visiting EBT Edge or by calling 1-888-328-2656.

Where can I find my case ID number?

Your case number can be found on all correspondence from us. It starts with "1B."

How can I apply for public assistance benefits?

You can apply for food, cash, medical and child care assistance, and RTD's LiVE transit program, at Colorado PEAK. There, you can also complete your recertification, or update your address and phone number, and report other changes.

Prefer to print it out? Download the food, cash and medical assistance applications in either English or Spanish.

What are the income limits for SNAP (food assistance)?

Most low-income households can get food assistance. To see if you qualify for SNAP benefits, check out the Colorado Department of Human Services website and complete an online pre-screening to see if you're eligible.

How do I get help using the PEAK website?

If you need help with a technical issue on the Colorado PEAK website, such as password resets, error messages, or problems with navigation, call the PEAK Technical Support Call Center at 1-800-250-7741.

How do I change my Health First Colorado health plan, get proof of coverage, or ask questions about billing?

For health plan enrollment, call 303-839-2120. For other questions, call Health First Colorado's member contact center at 1-800-221-3943.

How can I submit documents needed for my case?

You can upload documents on the Colorado PEAK website.

You can also leave them in a drop box at any of our three locations (listed below), fax them to 720-944-3094, or mail them to us at 1200 Federal Blvd., Denver, CO 80204.

We can also now accept client verification documents, in PDF format. Email them to us at DHSverifications@denvergov.org.

Civil Rights Plan and Policy

Denver Human Services (DHS) has created a Civil Rights Plan(PDF, 191KB) (updated October 2023). This plan was developed in collaboration with the Colorado Department of Health Care Policy and Financing (HCPF).
